Ready, Steady Go Programme

In healthcare, the word ‘transition’ is often used to describe the process of preparing, planning and moving from children's to adult services.

It can be scary to move away from a team of professionals that you have been with for many years but hopefully, by getting involved in the transition process, you will feel more confident and happier about the move. It will also help you understand your child’s condition better and support them to manage this more themselves.

The Ready Steady Go transition programme has been designed to help your child understand their condition and manage this as much as possible by themselves.

Who is it for?

Your child, supported by you, if they are over 11 with a long-term health condition such as diabetes or epilepsy.

What is it and why do I need to know?

A programme to help young people develop the knowledge and skills to manage their health condition as they move into adulthood.

Allowing young people greater control over their own condition improves their health outcomes in the long-term and improves their wellbeing. It will help young people gain the confidence and skills to move to adults services.

How?

Ask your child’s health team about developing a Ready, Steady, Go Transition Plan for the young person. They will be able to help you complete this
