Our vision for children and young people with special educational needs and disabilities is that they receive an education that enables them to achieve the best possible educational and other outcomes, and become confident, able to communicate their own views and ready to make a successful transition into adulthood, whether into employment, further or higher education or training.
Support needs to start early and should centre around the child or young person’s aspirations, interests and needs. It is really important that preparing for adulthood forms part of the planning for all children and young people with SEN and disabilities, right from the earliest years. However, for teenagers preparation for adult life needs to be a more explicit element of their planning and support. Discussions about their future should focus on what they want to achieve and the best way to support them to achieve.
The key transfer phases are early years provider to school, infant to junior school, primary to middle school, primary to secondary school and middle school to secondary school.
Where a child or a young person has an EHC plan and is moving between key phases, the plan is reviewed and amended in advance of the move, to allow planning for a successful transition to their new placement. The review and amendments are completed by 15th February for under 16’s , or by the 31st March for post 16 in the year of the transfer. All reviews taking place from Yr 9 onwards include a focus on preparing for adulthood, including employment and independent living The transition planning is a core part of the EHC plan and also includes planning for young people moving from children’s to adult social care and health services where relevant. These reviews are used to seek and record the views, wishes and feelings of the child or young person.
If the young person is nearing the end of their time in formal education and the EHC plan is likely to be ceased within the next 12 months, attendees at the review will work with the young person to help them take the next steps towards reaching their aspirations.
