Notification of Early Help

The local authority, together with partners across Education, Health, Care, Police, Housing and the voluntary & community sectors must work together to provide a cohesive Early Help offer to children and their families but we cannot know the extent to which agencies are providing Early Help unless we are notified.  Notifying us helps us link practitioners together if more than one agency is involved with the same child. This promotes information sharing and avoids unnecessary duplication.

  • The Notification - Monitoring Form is to be used to register all children/young people within your setting who are receiving support through the Early Help Graduated Pathway.
  • For Schools using SIMS: this form is to be used to record any child/young person receiving Early Help, who have not been recorded on your SIMS system. 
  • The Notification - Monitoring Form is to be used to notify the Early Help Partnership of all:
  • Existing CAF’s (which will need to be transferred to new paperwork at the next review)
    • New or existing My Plan
    • New or existing My Assessment and My Plan +
    • Closure of CAF (if you are transferring to new paperwork you will need to select the ‘Transfer to My Plan/Transfer to My Plan +’ option from the drop down menu so that it is clear support is still in place for the child/young person/family)
    • Closure of My Plan
    • Closure of My Assessment and My Plan +
    • Change of Lead Practitioner for a My Plan or My Assessment and My Plan +
  • If there is more than 1 Lead Practitioner in the setting please complete a Notification - Monitoring Form for each Lead Practitioner.
  • Please use this Monitoring Form to notify  your local Early Help Partnership of any updates or changes as needed throughout the year.
  • Please ensure that you have used a secure method of email to return this form. The preferred method is egress switch. If you do not have access to this please follow this link: Egress.
